The Transition to Adulthood Program at Children’s Mercy Kansas City is hosting a conference to support ALL patients 12+ and families as they prepare for adulthood on September 28th from 9am-2pm, breakfast and lunch will be provided. This annual conference is for patients, families and professionals working with transition aged youth. The focus of the conference this year is on developing life skills, managing health care, transition to college and much more. Attendees can expect engaging presentations, panels and breakout sessions. Community agencies will be on-site to provide needed resources and supports. The goal is to provide tools and resources to improve the transition to adulthood process.
